Weekly Planning Form
This allows a student to self-assess and reflect on their weekly performance, progress toward goals, as well as areas for improvement. It is signed by both the student and CI(s)

Anecdotal Record
Useful to document positive or negative behaviors during their clinical. Signed by both the student and CI(s)

Guidelines and Self-Assessments for Clinical Education
The intent of these voluntary guidelines is to provide academic and clinical educators with direction and guidance in the development and enhancement of clinical education sites and physical therapist and physical therapist assistant CIs and CCCEs. This comprehensive document includes three parts:

1.     Guidelines for Clinical Education Sites (including Self-Assessments for Clinical Education Sites)
2.     Guidelines for Clinical Instructors (including Self-Assessments for Cls)
3.     Guidelines for Center Coordinators of Clinical Education (including Self-Assessments for CCCEs)

Clinical Education Glossary of Terms
Can be used for discussion and description of physical therapist clinical education terms used by all stakeholders

Reference Manual for Site Coordinators of Clinical Education (SCCE) Manual
This manual was developed by Clinical Education Special Interest Group Leadership and Task Force to help SCCE and the PTs they work with to meet all of their clinical education goals

Clinical Site Information Form (CSIF)
The Clinical Site Information Form (CSIF) is a tool designed by the APTA for PT and PTA academic programs to collect information from clinical education sites. The CSIF is divided into two sections; Part I - Information for Academic Programs (pages 4-16) and Part II - Information for Students (pages 17-20). Completing the CSIF facilitates clinical site selection, assists in student placements, aids in assessing the learning experiences and clinical practice opportunities available to students and provides information required for accreditation.
